Complete the following (must be uploaded through registration form):

• Signed 2023 All-State Permission Form
• Signed and verified 2023 Academic Award Form if eligible
  (Students in grades 10-12 with a verified cumulative GPA of 3.5 or higher are eligible to receive this honor.)

You also must know:

• Your district number. If you’re not sure, click here for a list of parishes in each district.
• Your director’s email.
